This is such a hard question to answer. I'll just go with my gut, and the game I've probably played the most in my life: The Witcher 3: Wild Hunt.
It just has everything I like in videogames. Exploration, a dope narrative, memorable characters, deep customization, incredible music. Only real negative is the barely functional combat. I have to basically play on the hardest difficulty and not use a core mechanic to make the combat fun (I never use Quen) so I can't say it's a perfect game.
But it's close to it.
