this could mean venom is trying to lift the guilt of paz's suffering from himself and saying that she played part in the sufferings that came her way including how she died.
I'd say this part is maybe less about Venom trying to absolve himself of guilt and more him reminding himself of his failure, which is what the entirety of the Paz hallucination is. Ocelot says that the wound was all sealed up and in regards to the re-opening says "the medic has no idea. He figures it must have been self-inflicted." The use of "the medic's" opinion rather than Ocelot simply stating it seems purposeful given Venom's true identity. The medic wasn't able to assess the reality of Paz's wounds back in Ground Zeroes, and Venom is being reminded of that again here. Venom's guilt, his own mental wound, was in a way sealed away with his identity when he was made into Big Boss. The re-appearance of Paz is that wound re-opening, it's damage his psyche has self-inflicted.
I don't think Venom places any blame on Paz, even unconsciously. It's not in line with his characterisation.
