I genuinely think the breathing through skin thing was just shoehorned in after the 'you will regret your actions and deeds' tweet due to the backlash. Kojima is clearly a bit of a  pervert, we've seen scantly dressed women throughout the series as well as a few comments. E.G

• Sniper wolf's cleavage despite it being, what -20/30 degrees in Alaska?
  
• Meryl working out in her undies
  
• The locker posters MGS2, plus Fortune's get up
  
• The Boss' cleavage in MGS3 
  
• Beauty and the Beast unit was more tasteful IMO, but there was the photoshoot thing 
  
• Distracting guards with dirty mags etc
  

I'm sure there's more that I've missed too! None of the outfits are exactly realistic for the scenarios they find themselves in. Sure cleavage is nice to look at but they 'give you no tactical advantage whatsoever'  

These are just some things I - and probably everyone else - has noticed. I think he maybe pushed it a bit too far with Quiet and wanted to create some justification for it when he inevitably got the backlash as time has progressed.
